Lactation Services
Both Michelle Gerbi and Ami Burnham are in-network with Blue Cross Blue Shield, Regence, Premera, United Healthcare, and Pacific Source Commercial Plans.
We are also preferred providers for Oregon Health Plan members through Pacific Source Community Solutions. (This applies only to OHP clients covered under Pacific Source Community Solutions, not through other OHP carriers.)
We are not yet in-network with Washington State Medicaid programs but hope to be at some point in the near future.
Confirming Your Coverage
We understand that navigating insurance can be confusing, and we’re here to help make it easier.
If you schedule and complete your intake paperwork—including uploading your insurance information—at least 48 hours before your visit, our team will review your details and do our best to provide a Good Faith Estimate before your appointment.
If you’d like to verify your benefits before scheduling, please email hello@drgerbi.com with:
- Full names and dates of birth for both parent and baby
- Parent’s email address
- Photos of the front and back of each insurance card (for both parent and baby, if they have different plans)
- If coverage is under another family member, include their name and date of birth
Note for families with OHP through Pacific Source Community Solutions: Parent and baby are covered under separate plans, so we’ll need both cards to verify benefits correctly.
Newborn coverage tip: For the first 30 days after birth, most newborns are covered under the parent whose birthday falls earliest in the year. After 30 days, please confirm your baby has been officially added to your plan to avoid uncovered charges.
Billing and Payment for Lactation Visits
In most cases, both parent and baby are considered clients, and insurance is billed accordingly. This allows us to fully address feeding concerns for both of you and provide comprehensive, coordinated care.
Insurance coverage for lactation care can vary widely. Some plans cover lactation services for both parent and baby with no out-of-pocket cost. Other plans may require a copay, coinsurance, or deductible. Any applicable patient responsibility is collected at the time of service based on your individual benefits.
We are in network with select insurance plans and are often also able to bill out-of-network plans when appropriate. If we are unable to bill your plan directly, we are happy to provide a superbill for you to submit to your insurance for possible reimbursement.
Our team understands that insurance can feel overwhelming, and we’re happy to help. We’re often able to review coverage and answer questions over the phone prior to your appointment.
Time-of-service rates are available for families who choose to self-pay at their visit. Rates vary depending on the length and complexity of care. We accept HSA and HRA cards and can arrange payment plans when needed.

If you’re concerned about cost, please reach out—we can discuss alternative payment options or arrange a payment plan for families in need. We can also share information about community-based programs and local resources that may help cover or supplement care.

Scheduling & Cancellation Policy
We understand that life with a new baby can be unpredictable. If you need to reschedule, please let us know as soon as possible—ideally at least 24 hours in advance—to avoid being charged the full visit fee.
